  • Pudendal Neuralgia: A Missing Piece

    Pudendal neuralgia is one of the most intimidating diagnoses we treat.

    It’s complex, frustrating, and often misunderstood—even among pelvic health providers.

    💡 The posterior cutaneous femoral nerve.

    It’s easy to hyperfocus on the pudendal nerve itself, but the reality is:
    👉 It’s almost impossible for pudendal neuralgia to only involve the pudendal nerve.
    👉 This is why nerve blocks often fail to relieve all symptoms.
    👉 This is why symptoms frequently extend beyond the pudendal nerve’s pathway.

    If we don’t consider the posterior cutaneous femoral nerve, we may be missing a major driver of symptoms—and limiting our ability to help these complex patients.

    When treating diastasis recti, are we missing the real problem?

    💡 The back is often the missing piece.

    It’s easy to focus on the front—after all, that’s where the diastasis is! Most treatment plans go straight to engaging the transversus abdominus and strengthening the core muscles.

    But diastasis recti is often a symptom of a system problem—and that problem frequently starts in the back.

    In this episode, we break down:
    Why checking the obliques, paraspinals, and fascia of the back is critical
    How weakness or dysfunction in the back contributes to diastasis recti
    Why many patients can’t “stack their ribcage over their pelvis” (even if we tell them to!)
    How treating the back first often leads to better outcomes

    If we truly want to heal diastasis recti, we have to stop waiting until treatment isn’t working to assess the back—it should be part of the initial evaluation.

    Strength isn’t the key to fixing urinary incontinence.

    Welcome back to another episode from the Rising Vault—where we bring back some of our most impactful episodes to highlight The Missing Piece in common pelvic health conditions.

    Today, we’re tackling urinary incontinence—and the one thing that’s often overlooked:
    🟢 The focus on task and timing.

    Most of us were taught to assess voluntary pelvic floor strength first—but here’s the problem:
    👉 Pelvic floor strength isn’t correlated with incontinence.
    👉 A patient can test weak and have no symptoms at all.
    👉 A patient can test strong and still have massive incontinence issues.

    So instead of focusing on isolated strength, we need to ask:
    What task are they struggling with?
    When does the pelvic floor fail to meet the demand?
    How does the pelvic floor function in real-world situations?
