Despite the excellent skills and best intentions of ourstaff, occasionally things do not go as expected in the delivery of healthcare in our Hospital and Health Services. There are many review processes that can be used to assess and evaluate incidents that do occur. This episode discusses two of these processes, Part 6 clinical reviews and Part 9 health service investigations, which are available under the Hospital and Health Boards Act 2011, and explains the framework and points of difference for both these processes.
