There are many review processes that can be used to assess and evaluate incidents that do occur. This episode discusses two of these processes, Part 6 clinical reviews and Part 9 health service investigations, which are available under the Hospital and Health Boards Act 2011, and explains the framework and points of difference for both these processes.
