救急初期対応においては、初期評価Primary Surveyは特に力点を置いて訓練をしています。ただ1つこの場合だけは例外で、初期評価、Primary SurveyのABCDという順番を考えずに、とにかくAもBもCも同時に全部やらなければならない状態がある、あるいは初期評価の段階の前、患者に接触して第一印象の段階で別のアクションを起こさなければならない状態があるという説明をしています。
それは「心肺停止あるいはこれを疑う場合」です。大事なところは「これを疑う場合」も入っていることです。
意識がない、呼びかけても反応がないと、呼吸もどうやらしていないように見えるという場合は心肺停止が疑われます。
その場合は、初期評価のA、B、Cの順を辿っていくのではなく、心肺停止に対する心肺蘇生に直ちに入ることが大事です。そうでないと、心肺停止の場合だけは、救命のチャンスがなくなってしまいます
