 The Community Transformers Foundation (CTF) is a private independent foundation serving communities in the Commonwealth of Virginia. CTF was established to advance health and social equity. The Foundation seeks to address the social determinants of health, promote wellness and equality in communities that are historically underserved and underrepresented. CTF is exempt from Federal income tax as a 501c3 non-stock corporation. CTF was established from the for-profit agency Community Transformers LLC to serve marginalized communities in the Commonwealth of Virginia. CTF’s mission is to be an organization that is dedicated to building collaborations within communities to develop sustainable programs that address the social determinants of health and promote health equity.The Walk by P.H.A.I.T.H.
