L'Innominato, però, profondamente turbato dalla storia della giovane, si muove a compassione e decide di consegnare la ragazza al cardinale Federigo. Riunita alla madre Agnese, a Milano, Lucia viene scortata in un luogo sicuro mentre il cardinale le promette di fare di tutto per rintracciare Renzo, il suo promesso sposo.A Milano esplode una grossa epidemia di peste che miete migliaia di vittime. Contagiato in forma lieve dal morbo, Renzo si reca al lazzaretto dove incontra Fra' Cristoforo che soccorre gli infermi. Il buon padre accompagna il ragazzo in una capanna dove ritrova il moribondo Don Rodrigo, a cui perdona il male subìto. Ma non tutto è perduto: tra le convalescenti Renzo riconosce Lucia, i due promessi si ritrovano finalmente liberi e guariti dalla peste, eppure non possono ancora sposarsi: sulla ragazza pesa il voto di castità fatto durante la notte di prigionia al castello dell'Innominato.
