Hans dokumentarfilm "De gamle" (1961), "Familiebilleder" (1964) og "Ung" (1965) fremstår som hovedværker i moderne dansk dokumentarfilm med deres nuancerede skildringer af livsformer og holdninger i 1960'ernes danske velfærdssamfund. I 1966 fik Carlsen et internationalt gennembrud med sin betydeligste film "Sult" en filmatisering af Knut Hamsuns roman af samme navn.
