Franz Kafka connut d'abord Milena comme traductrice : elle établissait la version tchèque de quelques-unes de ses proses courtes. Ces relations se transformèrent en une liaison passionnée dont les lettres permettent de suivre le progrès. Cette passion ne dura qu'un instant, elle tient en quelques mois à peine.
Les lettres racontent d'un bout à l'autre ce roman d'amour, orgie de désespoir et de félicité, de mortification et d'humiliation. Car quelle qu'ait pu être la fréquence de leurs rencontres, leurs amours restent essentiellement épistolaires comme celles de Werther ou de Kierkegaard.
Milena est morte vingt ans après Kafka, dans le camp de concentration de Ravensbrück.
