One is by asking which rule was broken, who did it, whether that behavior crossed some line, and what the appropriate consequences should be. In this retributive sense, an account is something you get people to pay or settle. But who will draw that line? And is the process fair? Other ways to approach accountability after an incident is to ask who was hurt; to ask what their needs are; and to explore whose obligation it is to meet those needs. People involved in causing the incident may well want to participate in meeting those needs. In this restorative sense, an account is something you get people to tell and others to listen to.

Just culture delivers

Sound reasoning and ethics drawing on real life examples from many safety critical areas make this book a compelling resource to safer and fairer operations. Highly recommended for practitioners and operators but most of all for those in roles of managing and regulating safety critical operations.

culture influence on investigations

I listened to both books "Just Culture: restoring trust.." and "Field guide.." by Sidney Dekker. I recommend starting with this book because it is easier to understand and contains many good stories. Also, the writer expands on "Culture" in the filed guid book.

Sidney Dekker shines a light of hope and reason into an area where unjust witch hunts and over eager enforcement have darkened our world. He is one of a handful of inspirational voices currently changing the H&S climate. Well worth a listen.
