의사 심영빈은 교사인 부인과 두 딸, 어머니와 함께 살고 있는 40대의 가장이다. 그의 위로는 가장의 짐을 벗고 미국으로 떠나 돌아오지 않는 형님, 밑으로는 대기업 회장의 장남인 경호와 결혼한 여동생 영묘가 있다. 영묘의 남편 경호의 건강이 심상치 않자 영빈은 환자에게 병세를 알릴 것을, 경호의 집안에서는 환자에게 숨길 것을 강요한다. 경호는 자신의 병명도 모르고 이렇다 할 병원 치료도 받지 못한 채 대체의학이라는 미명 하에 주술적 치료를 받게 된다. 한편 영빈은 중학교 때 첫사랑 현금과 재회해 미묘한 관계를 이어나간다.

박완서는 『아주 오래된 농담』의 주인공, 40대의 의사 심영빈의 시선을 통해 자본주의의 속내를 뒤집어 보고자 했다. 환자에게 병세를 알리지 않은 것이 돈과 결탁된 눈가림이었다는, 그것이 생명과 감정보다 돈을 우선시하는 현대인이 추종하는 자본주의의 실상임을 낱낱이 파헤친 작품이다.
